Vivelle Dot Information
Vivelle Dot is an estrogen (transdermal patch) drug class medication used to treat moderate to severe symptoms of menopause, such as hot flashes, night sweats, vaginal dryness, and vulvar or vaginal atrophy. It is also indicated to treat low estrogen levels caused by hypogonadism or primary ovarian failure, and to prevent postmenopausal osteoporosis in women at high risk of bone loss. It works by providing a steady, controlled release of estradiol, the primary estrogen naturally produced by the ovaries, through the skin and directly into the bloodstream, replenishing declining hormone levels. Because it is a transdermal therapy, it avoids first-pass metabolism in the liver, allowing for a lower dose of hormones to be used compared to traditional oral pills.
Side Effects
Commonly reported side effects include application-site reactions, such as redness, irritation, or itching at the patch site. You may also experience headaches, migraines, breast tenderness or pain (mastalgia), back pain, bloating, abdominal pain, nausea, and minor weight gain. Breakthrough bleeding, spotting, changes in menstrual periods, and localized vaginal irritation or discharge are also relatively frequent as the body adjusts to the hormones.
Contact your doctor if you have any of the serious side effects including:
• Cardiovascular Events: An increased risk of developing serious blood clots (such as deep vein thrombosis or pulmonary embolism), a stroke, or a heart attack.
• Gallbladder Disease.
• Malignant Neoplasms: Increased long-term risk of developing endometrial cancer of the uterine lining if used without a progestogen, as well as a potential risk of breast or ovarian cancer.
• Severe Eye Changes: Potential for vision loss or structural changes in the eye due to retinal vascular thrombosis.
• Severe Hypertension
Seek medical attention immediately if you experience abnormal or unexplained vaginal bleeding, discover new lumps or skin dimpling in your breast, or notice sudden swelling, pain, or warmth in one leg. Get emergency care right away if you experience sudden severe chest pain, coughing up blood, sudden shortness of breath, a sudden severe headache, confusion, difficulty speaking, or yellowing of the skin and eyes (jaundice). Do not stop using this medication suddenly if you are on a long-term regimen without consulting your healthcare provider first, as a sudden withdrawal can cause a rapid return of your menopausal symptoms or trigger irregular bleeding.
Precautions:
Do not use Vivelle Dot if you have a known hypersensitivity or serious allergic reaction to estradiol or any inactive components in the patch matrix or adhesive. Tell your doctor about your medical history, especially if you have:
o Coronary artery disease, high blood pressure, hereditary angioedema, or a history of migraines.
o Current, suspected, or historical breast cancer, uterine cancer, or other estrogen-dependent malignancies.
o History of, active, or suspected blood clots in the legs or lungs (DVT or pulmonary embolism).
o Known or suspected pregnancy.
o Liver disease or impaired liver function.
o Recent heart attack, stroke, or transient ischemic attack (TIA).
o Undiagnosed or unexplained abnormal vaginal bleeding.
Vivelle Dot may interact with medications that induce liver enzymes, such as carbamazepine, phenobarbital, rifampin, and St. John's Wort, which can speed up hormone clearance and reduce the patch's clinical effectiveness. It counteracts the therapeutic effects of aromatase inhibitors (such as anastrozole, letrozole, or exemestane) and should not be used concurrently, as these classes have opposing mechanisms of action. It may also interact with certain antibiotics (such as erythromycin) or antiviral treatments, and can alter the required dosage of thyroid replacement therapy.
Vivelle Dot is strictly contraindicated during pregnancy and must be removed immediately if pregnancy is suspected, as it can cause fetal harm. Estrogens pass into human milk and can reduce the absolute quantity and quality of milk produced. Therefore, use during lactation is generally not recommended. It is recommended to speak to your doctor immediately if you are planning to get pregnant.
For postmenopausal women who still have an intact uterus, Vivelle Dot must be paired with a progestogen as a standard clinical requirement to counteract the stimulating effect of estrogen on the uterine lining and minimize the risk of endometrial hyperplasia or cancer.
Store Vivelle Dot patches at room temperature away from excessive heat and moisture. Keep each patch securely sealed inside its individual, original protective pouch until immediately before application. After removing an old patch, fold it in half with the sticky sides facing each other and dispose of it safely in a sturdy container out of reach of children and pets, as used patches still contain active medication.
